Notifications sent out after Alberta health screening questionnaires misplaced

CALGARY – Canadian Blood Services says records from southern Alberta containing health screening questionnaires involving more than 2,600 people have gone missing.

The non-profit charitable organization that manages the national supply of blood, blood products and stem cells says it was notified late last month by a third party which stores the records that two boxes of documents have been misplaced.

Blood services believes the boxes are likely still somewhere within a storage facility.

The questionnaires were completed in 2006 and 2008 to help assess whether the respondents were able to donate.
