Notifications sent out after Alberta health screening questionnaires misplaced

CALGARY – Canadian Blood Services says records from southern Alberta containing health screening questionnaires involving more than 2,600 people have gone missing.

The non-profit charitable organization that manages the national supply of blood, blood products and stem cells says it was notified late last month by a third party which stores the records that two boxes of documents have been misplaced.

Blood services believes the boxes are likely still somewhere within a storage facility.

The questionnaires were completed in 2006 and 2008 to help assess whether the respondents were able to donate.

Everyone affected by the possible privacy breach is being notified.

Since July 2016 all records have been digitized a move which the organization says has significantly reduced the chance of this type of incident recurring.
